Our Approach
Led by Dr. Caryn Blair, DNP, APRN, PMHNP-BC — a Doctor of Nursing Practice–prepared clinician with over 30 years of nursing experience — every step of our process reflects both the depth of advanced nursing practice and the human reality of what caregiving actually feels like from the inside.
Most families navigating Alzheimer's or dementia care don't lack love or dedication. What they lack is clarity — a structured, clinical framework that helps them understand what's happening, what to expect, and how to keep moving forward without losing themselves in the process. That's exactly what our approach is designed to provide.
Step One
Before any assessment, before any care plan, we create space for a real conversation. Not a rushed intake. Not a form to fill out alone in a waiting room.
We want to understand who your loved one is beyond their diagnosis. We want to know what caregiving has looked like for you — the exhaustion, the grief that comes in waves, the moments of unexpected tenderness, and the days that feel unsurvivable.
This isn't a formality. It's the foundation. What we learn in this conversation shapes every clinical decision that follows.
"You've spent a long time making sure everyone else feels heard. This is where someone finally does the same for you."
Step Two
Once we understand your situation, our DNP-prepared Psychiatric-Mental Health Nurse Practitioners conduct a thorough, structured nursing assessment — one that looks at the full picture rather than isolated symptoms.
We evaluate the behavioral and cognitive patterns your loved one is experiencing. We look at the specific pressures you're carrying as a caregiver. We examine how those two realities are intersecting — and where clinical support can make the most meaningful difference.
What we're building is clarity. Because when you understand what's actually happening — clinically, behaviorally, emotionally — you stop reacting in the dark and start making decisions with confidence and purpose.
Every assessment is grounded in current nursing evidence-based practice. Nothing is assumed. Everything is evaluated with care and precision.
Step Three
Most care plans are designed for the patient and handed to the caregiver as a set of instructions. Ours are built differently — with you as an active, informed participant in the process.
Because here's what the research consistently shows: your capacity, your wellbeing, and your understanding of the situation are not secondary concerns. They are clinically significant. What happens to you directly affects the quality of care your loved one receives. So you belong in the room where the plan is made.
This is caregiver-inclusive care planning. Not directives. Not a pamphlet. A living plan that belongs to both of you.
Step Four
Alzheimer's and dementia are progressive conditions. The behavioral and cognitive challenges your loved one faces today will look different in six months, and different again after that. What you need from a care team will shift — and our supportive nursing services are designed to shift with you.
This is where the clinical relationship really takes root. Our Psychiatric-Mental Health Nurse Practitioners stay with your case over time — getting to know the full arc of your journey, not just a single snapshot of it. When something changes, we recalibrate. When something gets harder, we're already there.
You won't have to re-explain your situation every time you need support. You won't be handed a resource list and told to figure it out. You'll have a clinical presence that is ongoing, responsive, and genuinely invested in how this unfolds for your family.
The Principles Behind Everything We Do
Every recommendation we make is grounded in current nursing research and clinical standards. We bring the science and the humanity together — because good care requires both.
The way families experience illness, navigate difficult decisions, and communicate about care is shaped by culture, history, and relationship. We approach every family with genuine curiosity and respect.
When caregivers receive structured, qualified support, the people they care for do better. Your wellbeing is not a nice-to-have. It is clinically central to the work we do together.
Who We Serve
If you are caring for a loved one with Alzheimer's disease or a related neurocognitive condition and you are experiencing stress, uncertainty, or decision fatigue, you are exactly who this practice was built for. You don't need to be in crisis to deserve support. You just need to be in it.
If you work alongside families navigating dementia care and are looking for a trusted, clinically credible nursing-led resource to refer to, we welcome that conversation. Our structured approach and DNP-level clinical leadership make us a reliable partner for professionals seeking high-quality support for their clients and patients.
